Alzheimer's Disease & Sundowning

The Alzheimer’s Association explains, “sundowner’s syndrome” or “sundowning” is a set of symptoms or behaviors that are found in some people with Alzheimer’s disease and dementia. The syndrome’s symptoms include agitation and anxiety, and it occurs in the later part of the day and evening with marked behavior changes in the person.

Fred Kobylarz, MD, co-director of the Center for Healthy Aging at Rutgers Robert Wood Johnson Medical School says it is not a disease, but rather a symptom of dementia. While doctors do not know the cause of sundowning, they believe it may be a “combination of hormonal changes, brain deterioration or damage that has occurred, environmental factors, disruption to a person’s circadian rhythm, and more.”

The syndrome may start as early as 1 p.m. in some people but usually is later in the day. Along with agitation and anxiety; symptoms can include things like pacing, disorientation, hallucinations and problems sleeping. Other behavior changes may include angry outbursts and aggressive, sometimes even physical altercations. Others just want to be closer to a person they trust as needed reassurance. Some people do not remember what happened earlier in the day and even forget they just ate dinner and want to eat again. 

Experts say in some cases sundowning starts before a person has had a diagnosis of Alzheimer’s or another form of dementia. However, it is not only found in people with dementia as some older adults exhibit symptoms during an illness or period of delirium.    

Doctors recommend families who see symptoms of sundowning to consult with their healthcare provider to rule out other causes. Guidelines for managing the syndrome include setting up a schedule that adheres to their needs and maintaining a consistent daily routine to reduce confusion and fatigue. During morning hours encourage the person to do activities that increase their exposure to natural light to help support better sleep-wake cycles. Avoid over stimulating the person in the evenings and don’t subject them to crowds, bright screens or loud noises.

Do You Need Blue Light Blocking Glasses?

Blue light is a high-energy wavelength of visible light, which can be emitted from our digital screens that we all seem to be using more and more often. To “protect” us from this light, there are a plethora of blue light protected glasses everywhere. In addition, we are hearing how much we need them now more than ever. So the question is do they actually work and is the extra expense worth it? 

Since 2008, researchers have been studying blue-blocking or filtering products that are trying to decrease the transmission of ultraviolet light with wavelengths between 440 and 500 nanometers. Some digital devices recently offer blue light-filtering settings to reduce the transmission of short wavelengths of light.

However, it may not surprise you to hear there is some debate on whether we are adversely affected by blue light, and to what degree blue light glasses can help the problem. The Mayo Clinic explains, while blue light occurs in daylight, the waves omitting from our devices at night could be detrimental to a good night’s sleep as they may shift our circadian rhythm, the body’s internal clock. However, they also say studies do not show an improvement in eye strain or better sleep quality by using blue-blocking glasses. The American Academy of Ophthalmology agrees and doesn’t recommend special eyewear for computer users.

Instead experts offer some suggestions to help diminish eyestrain and improve sleep such as monitoring the length of your screen time and taking frequent breaks. They say try the 20-20-20 rule of looking at an object 20 feet away for 20 seconds every 20 minutes. In addition, an updated eye exam could detect problems with your eyes and whether prescription glasses might help eyestrain.

Using a Walker Can Lower Fall Risk

If you are at the point in life where you are not as stable as you used to be, have balance problems, arthritis or leg weakness; a mobility aid such as a walker could help keep you moving and lessen your chance of a fall. The first thing to do is work with a physical therapist or your healthcare provider to discuss the best type of walker for your personal needs and comfort. There are several fit points to consider as well as styles.

One common choice is a standard walker with four rubber-tipped legs and no wheels. Experts say it provides the most stability but has the drawback of having to pick it up to move. Other choices include walkers with wheels. A two-wheel walker has wheels on the front and can be helpful for those who have trouble picking up a standard walker. Four–wheel walkers may help those very unsteady on their feet but can be a bit less stable. This type of walker has the option of a seat for those that need to rest often. 

Another important step in getting the proper walker is the height of the arms. The Mayo Clinic suggests stepping into the walker with your shoulders relaxed and upright. Elbows should be able to comfortably bend at 15 degrees. Other tips include remembering to stand upright when using them as it is easier on your back, and to move one step at a time. Step into the walker with each step rather than walking behind it. 

Use caution when using a walker on a slippery or wet surface, and even carpet requires extra care. Just as you do when you are walking on your own, be sure to wear sensible low-heeled shoes that fit well and have good grips on the soles.

Add-ons and various options make walkers more convenient. Fold-up models allow them to be easily transported. Attachable bags and baskets ensure you can carry items you need while keeping hands-free to concentrate on safely moving the walker. Other options include trays for drinks or meals and seats.

Muscle Loss in Older Adults

The term sarcopenia refers to age-related muscle loss and is a natural part of growing older. However, it also may occur at a faster pace after prolonged periods of inactivity from injury, illness or the pain of osteoarthritis. Sarcopenia causes muscle atrophy and brings on problems such as poor balance and greater weakness, which in turn, can result in more inactivity and increased risks for falls.   

Seniors with muscle atrophy may have difficulty reaching their arms upward, opening jars, typing on a keyboard or even tying shoes.  The Journal of Rehabilitation Medicine reports on a study that found “older men who did eight weeks of strength training lost about 25% of their muscle gains after they stopped training for two weeks.”

Jodi Klein, a physical therapist with Harvard’s Brigham and Women’s Hospital, suggests a few things you can do if you know you have a surgical procedure ahead of you that could cause muscle atrophy. She says if you are currently active, keep it up; if not, use this time to get started.  Klein explains, “The better shape you are in going into surgery, the better shape you will be coming out, and the less chance you have of enduring muscle loss.” 

Older adults who want to regain muscle strength should talk to their health care provider about the best methods. Experts recommend an exercise routine that builds both upper and lower body strength. This could include weight training with dumbbells and resistance bands in conjunction with walking, swimming, and biking. If it would be helpful, think about turning to an expert to aid you in regaining strength.

Do You Need a Vitamin Supplement?

As we age, it becomes more important than ever to protect our health but making decisions on how we do it can be confusing at best. One day it seems this diet is better than another and they have lots of initials such as DASH, MIND, TLC or KETO as well as Mediterranean, vegan, vegetarian and flexitarian diets. Adding to the challenge is whether we need a daily multivitamin. 

While it is a good idea to get advice from your healthcare provider, it doesn’t hurt to hear what some other experts say. For example, the Harvard School of Public Health says if you eat a healthy diet, a multivitamin may have little or no benefit. A healthy diet should include plenty of vegetables, fruits, whole grains, good protein sources and healthy fats. 

However, the National Academy of Medicine cautions some Americans may not be getting enough of specific vitamins and minerals such as vitamin D and vitamin E from food sources alone. Vitamin deficiency can also occur in people with diseases like celiac, ulcerative colitis, or cystic fibrosis, which cause their digestive systems to not function properly. Others who may benefit from the addition of specific vitamins include the elderly or conditions such as pregnancy, alcoholism and taking certain medications including treating high blood pressure. 

Doctors say even those with diseases such as osteoporosis may only need to increase their intake of nutrients targeted toward maintaining bone health, or if that is not possible to take a supplement. If in doubt, ask your healthcare team if you should consult with a dietician. Remember to let your doctors know about any supplements you are taking in case of drug interactions with prescription medications.
